ANIMAL CONTROL COMPLAINT

To the Complaintant: NH RSA 466:31 specifies that a complaint regarding a dog which is considered to be a nuisance, a menace, or vicious be in writing. By filing out and signing this form, you are requesting that an investigation proceed and agree to cooperate in the event that a prosecution in court is undertaken.  This form must be printed or typed.
